Lipoedema Stage 4: When Lymphoedema Is Added

In shortStage 4 is advanced lipoedema with associated lymphoedema (lipo-lymphoedema). On top of volume and pain there is fluid retention, and the foot is no longer spared. It needs both treated: lymphatic control first, surgery afterwards and always in stages.

What makes it different from stage 3

  • The foot and its dorsum are affected, so the cuff sign disappears.
  • Pitting oedema: pressing leaves a dent for a few seconds.
  • It can be asymmetric, unlike pure lipoedema.
  • Recurrent infections (cellulitis) from lymphatic compromise.

Why the order matters

At stage 4 you do not operate before the lymphatic component is under control. Operating on a decompensated lymphoedema worsens the result. The usual sequence is decongestive therapy, stabilisation, then staged surgery with WAL.

Is surgery worth it at stage 4

Yes, with the right expectations. The goal is not a cosmetically perfect leg: it is less volume, less pain, fewer infections and regained independence. For many patients that means walking without rubbing and sleeping without pain.
